Employees ordered to report to, or who volunteer for, active duty in the National Guard or Reserves or Armed Forces should notify their supervisors as soon as possible and present them with a copy of their orders as soon as they are received.
The supervisor should attach a copy of the orders to the completed Leave Request Form [PDF] and submit to the Office of Human Resources.
