Reduced Medical Rates: Schedule B

Benefits-eligible employees enrolled in the Syracuse University health plan automatically pay the Schedule A rates.  Depending on your household income and size (see the chart below), you may be eligible for lower health plan contributions under Schedule B, with the exact same benefits and coverage as provided under Schedule A rates. You must apply and qualify for reduced rates. Certain employees represented by the SEIU collective bargaining agreement are automatically enrolled in the Schedule B rates.

Eligibility

Eligibility for this program is based on household income and household size.

For 2024, the criteria is as follows:

Household size is the number of individuals declared on your Federal Income Tax Return, and if applicable, your spouse or domestic partner. Also included in household size are your children who are not declared on your tax return, but who are either under age 19 and living with you, or age 19 or older and enrolled as your dependent in the medical plan.

Household income is the combined adjusted gross income disclosed on your Federal Income Tax Return, including for your spouse or domestic partner, as well as your children age 19 or older if they are enrolled on your medical plan. The adjusted gross income is on the Federal Income Tax Return (line 11 of Form 1040).

Application Process

If you are a new employee, applications must be submitted within 31 days of your hire date. For current employees, the Schedule B application period for 2024 has closed. The deadline to submit the application for reduced rates for 2024 was Dec. 1, 2023. If you recently entered the United States and did not file a 2022 US Federal Income Tax Return, please contact HR Shared Services for additional information on how to submit your Schedule B Application.The Schedule B Reduced Medical Contribution Application is an eForm.  Our eForms are secure, fast, and accessible.To access the application in your MySlice portal, follow these easy steps:

  1. Log into your MySlice portal.
  2. Under “Employee Home” select the “Employee Resources” tile.
  3. Within the “Employee Resources” tile select the “eForms” tile.
  4. Expand the “Human Resources” section by clicking on the grey triangle on the left side of the page.
  5. Select the appropriate Schedule B application from the list to be taken to the application:
    1. If you are a current employee applying for Schedule B during Open Enrollment, select “Open Enrollment Schedule B App” from the list.
    2. If you are a new employee applying for Schedule B within 31 days of your hire date, select “New Hire Schedule B App” from the list.
  6. Complete the application by answering all the questions and securely uploading any required documentation.
  7. Ensure that you click the “Submit” button at the bottom of the application to send your completed application to HR Shared Services for review. Upon submission, a confirmation email will be sent to your Syracuse University email.
  8. If you are submitting the application during your new hire period, communication will be sent to your Syracuse University email regarding your application status once reviewed.

You must re-apply each year for Schedule B Medical Rates.
