Choosing health coverage is an important decision. To help you make an informed choice, a Summary of Benefits and Coverage (SBC) is available to you. The SBC summarizes important information about health coverage in a standard format to help you compare features across coverage options available to you. Syracuse University’s SBCs are updated annually and are posted on the Human Resources website at the beginning of each annual open enrollment period for coverage beginning Jan. 1. You have the right to receive a copy of the SBC at any time. If you wish to receive a paper copy of the SBC, please contact HR Shared Services at 315.443.4042. Please share this information with your family members who are eligible for or enrolled in plan coverage. The Syracuse University Group Health Plans Notice of Privacy Practices (NPP) summarizes the privacy practices of the group health plans. You have the right to receive a copy of the NPP at any time. If you wish to receive a paper copy of the NPP, contact HR Shared Services at 315.443.4042. Please share this information with your family members who are eligible for or enrolled in plan coverage. The medical plan, as required by the Women’s Health and Cancer Rights Act of 1998, provides benefits for mastectomy-related services, including all stages of reconstruction and surgery to achieve symmetry between the breasts, prostheses and complications resulting from a mastectomy, including lymphedema. Syracuse University’s Women’s Health and Cancer Rights notice is available on the Syracuse University Policies website. Please share this information with your family members who are eligible for or enrolled in plan coverage. Notice Regarding the Syracuse University Summary of Benefits and Coverage
