Others may apply if you meet income and household size requirements.<\/p>\n      <\/div>\n    <\/div>\n      <div class=\"accordion-item border border-gray-light\">\n      <div class=\"accordion-header-text\">\n        <h3 class=\"margin-bottom-0\">\n          <button class=\"accordion-header full-width background-gray-light-30 border-0 margin-0 padding-top-2 padding-bottom-2 padding-left-3 padding-right-3\" aria-expanded=\"false\">\n            Reduced Health Plan Contributions for SEIU Members            <div class='accordion-toggle icon-right' class=''>\n              <i class='icon-chevron-right'><\/i>\n            <\/div>\n          <\/button>\n        <\/h3>\n      <\/div>\n      <div class='accordion-content'>\n        <p class=\"whitespace-normal break-words\"><strong>SEIU Legacy Members: Automatic Schedule B Eligibility<\/strong><\/p>\n<p class=\"whitespace-normal break-words\">SEIU Legacy members meeting the following criteria automatically qualify for lower health plan contributions under Schedule B:<\/p>\n<ul class=\"[&amp;:not(:last-child)_ul]:pb-1 [&amp;:not(:last-child)_ol]:pb-1 list-disc space-y-2.5 pl-7\">\n<li class=\"whitespace-normal break-words\">Classified as Grade Two or Three<\/li>\n<li class=\"whitespace-normal break-words\">Completed at least two years of service by<strong> Dec. 31, 2027<\/strong><\/li>\n<\/ul>\n<p class=\"whitespace-normal break-words\"><strong>What to Expect:<\/strong><\/p>\n<p class=\"whitespace-normal break-words\">When you complete Open Enrollment in MySlice, <strong>Nov. 2\u201313<\/strong>, you&#8217;ll see contributions based on your current enrollment. In mid-December, HR Shared Services will adjust rates for qualifying SEIU members and send a revised confirmation statement to your Syracuse University email.<\/p>\n<p><strong>Other SEIU Members May Still Apply:<\/strong><\/p>\n<p>If you don&#8217;t automatically qualify, you can still apply for lower contributions through Schedule B. <strong>Apply by Dec. 4<\/strong> in <strong>MySlice <\/strong>under <strong>Employee Resources &gt; eForms &gt; Human Resources<\/strong>.<\/p>\n<p>Visit the <strong><a href=\"https:\/\/hr.syr.edu\/benefits\/health-benefits\/medical\/reduced-medical-rates-schedule-b\/\">Schedule B webpage<\/a><\/strong> for more information.<\/p>\n      <\/div>\n    <\/div>\n      <div class=\"accordion-item border border-gray-light\">\n      <div class=\"accordion-header-text\">\n        <h3 class=\"margin-bottom-0\">\n          <button class=\"accordion-header full-width background-gray-light-30 border-0 margin-0 padding-top-2 padding-bottom-2 padding-left-3 padding-right-3\" aria-expanded=\"false\">\n            SEBF Dental and Vision Plan            <div class='accordion-toggle icon-right' class=''>\n              <i class='icon-chevron-right'><\/i>\n            <\/div>\n          <\/button>\n        <\/h3>\n      <\/div>\n      <div class='accordion-content'>\n        <p>If you are a member of the SEIU Legacy bargaining unit, you may choose to enroll in the<span>\u00a0<\/span><strong><a href=\"https:\/\/hr.syr.edu\/benefits\/health-benefits\/dental-and-vision\/sebf-dental-and-vision\" rel=\"noopener\">Service Employees Benefit Fund (SEBF) dental and vision plans<\/a><\/strong><span>\u00a0<\/span>instead of the University plans. If you have specific questions regarding the plans through SEBF, please contact SEBF at<span>\u00a0<\/span><strong>855.835.9720<\/strong>.<\/p>\n<p><strong>Remember:<\/strong><\/p>\n<ul>\n<li><strong><strong>If you are enrolled in one of the University\u2019s plans:<span>\u00a0<\/span><\/strong><\/strong>This is the second year of a two-year commitment. The premium for this coverage will be fully employee paid through payroll deduction.<\/p>\n<p>To enroll, please complete the SEBF Enrollment\/Payroll Authorization Form and return it to SEBF any time prior to\u00a0<strong>Dec. 13<\/strong>.<\/p>\n<p><strong>The SEBF Weekly Disability Plan offers:<\/strong><\/p>\n<ul>\n<li>A benefit of one-half of your average weekly salary to a <strong>maximum of $170 per week<\/strong> for <strong>up to 26 weeks<\/strong>.<\/li>\n<li>A weekly benefit that is paid for any injury or illness that is not work related.<\/li>\n<li>No premium payments are required while you are out of work due to an approved disability.<\/li>\n<li>You can collect the SEBF benefit in addition to other disability benefits, such as New York State Statutory Disability and Syracuse University Supplemental Short Term Disability.
